Fungal Nail Initial Assessment & Treatment

An initial assessment includes a thorough appraisal of your medical history and a neurovascular review; where your pedal (foot) pulses will be checked with handheld Doppler ultrasound (this is non-invasive) and nerve function professionally assessed. We will discuss the treatments you have tried already, if any, and address any concurrent skin conditions.
The appearance of your nails will be hugely improved in just one treatment.
A management plan will be discussed and all treatments are completed with the application of an hydrating Gehwol® foot spray and relaxing foot massage.

A fungal nail sample can be sent for laboratory testing, if indicated. Please see options below. The pros and cons of each will be discussed with you during your appointment.
Treatment includes: -
- reduction of the symptomatic nail/s to improve aesthetic appearance and allow any topical treatments to penetrate the nail plate to reach the nail bed.
- Both feet are treated with all non-infected toenails treated first, to prevent cross-contamination.
- sample taken for either lab (see below) or in-clinic test using a dermatophyte test strip
- fenestration (tiny holes precisely drilled into the nail bed); indication for this assessed on a case-by-case basis

Anti-fungal medications will incur an additional charge.
You may be eligible for Toenail Reconstruction to conceal your nails during treatment to give a more elegant appearance.
A summary of advice discussed and/or a management plan will be emailed to you after your appointment.
An initial assessment is 45 minutes, with follow-up appointments typically lasting 30 minutes (£55). A 45 minute follow-up appointment is also available if you've not been seen for a while or require a little extra TLC.
Additional Charges
First line medicaments such as a broad-spectrum anti-fungal cream for athlete's foot, a moisturising foot cream for dry feet, a basic deflective insole etc will incur an additional charge. If a referral or letter is required and cannot be completed during your appointment, this may incur a further £15 - £30 admin fee depending on time required to complete the task/s.
Testing for Fungus
Typically, I will recommend that a fungal nail sample is taken to confirm diagnosis.
The options are:-
i) Microscopy & Culture via your GP (no charge; please note, not all GP practices will accept a sample from a private podiatrist) or Alan James Clinics (fees charged by AJC).
ii) PCR requested privately via The Doctors Laboratory (fee £75)
iii) In-clinic DTS with letter to GP (fee £34.99 if undertaken during Initial Appointment; further charge if performed during a follow-up appointment)

Depending on the outcome of the test, you may be eligible for an oral antifungal tablet. This can be requested via your GP or arranged privately via Alan James Clinics. We will discuss the options in detail during your appointment.

For follow-up appointments, all additional devices (e.g. Bland-Rose toe props, removable felt props, animal wool, heel pads), insoles, creams and some medicaments (e.g. silver nitrate) will incur nominal further charges.
